Each insurance plan has different reimbursement guidelines for coverage for your products. At Xceed Medical we gather your insurance information prior to your appointment or order and we attempt to get a commitment from your insurance carrier. Once we fit you and you are satisfied with your product, Xceed Medical will bill insurance on your behalf, and only after we have resolved the insurance balance will you potentially receive an owed balance, limited by maximum’s established before product(s) is placed with the patient.

If you were involved in a Workers’ Compensation injury that was filed and accepted by your employer, then the insurance carrier for your employer will pay for all charges associated with your product.

All Xceed Medical products are dispensed only under direct instruction from a physician. Please have your physician contact us at 414.235.8552 or fax over a prescription to 262.258.2855.

We have received an order for product(s), generally this is for an upcoming procedure or for any questions that we may have on an existing order. We call our customers to verify insurance and delivery information as well as to go over insurance benefits and arrange your fitting. Please call us back at 414.235-8552 during our business hours so that we make sure all information is up to date.

At the time you purchase any product(s) from Xceed Medical, we will advise you of an estimated cost. After you have completed the purchase of Xceed Medical product(s), we submit charges to your insurance. Your insurance will adjust the charges to reflect how much is allowed to be paid, and how much is not covered per your benefit plan. Through a document called an Explanation of Benefits (EOB), your insurance will notify Xceed Medical and you of 1) how much is allowed for each product you received or if the product is not covered, 2) how much the insurance paid for each product, and 3) how much you are responsible for. Xceed Medical will review the EOB to ensure the accuracy of your insurance’s decision, which allows us to honor the cost estimated to you at the time you received the product. We will then send a bill to you that will reflect charges for copay, co-insurance, deductibles, or not covered products.
